Below are the terms and conditions on which we (Abi Bickley - Children’s Physiotherapy) offer our services. Please take the time to read through these terms and conditions, and if you would like anything further explained, please do get in touch.
- Your child will be examined and treated in line with the Chartered Society of Physiotherapy's strict code of practice by a qualified Physiotherapist, Registered with the Health Professions Council.
- All personal information is stored and kept strictly confidential.
- All patient related information is taken in the strictest confidence. However, in the event of disclosure which highlights risk to the patient or others, we will be required to pass the information on to the relevant professionals/organisation.
- For legal reasons, patient information will be held until eight years from the date of last treatment for adult records, and for children eight years after their 18th birthday or until 25 years of age.
- Access to medical records - We agree to supply copies of records to you or a third party subject to receipt of the request in writing with your full signature. We are registered with the Information Commissioner.
Initial Assessment:
An Initial Assessment takes approximately 60 minutes and will involve;
-A discussion of your needs with us.
-Talking about your past medical history.
-Information gathering on the history of the condition/issue.
-A full objective examination of your problem.
-Advice and any recommendations.
-A written summary of the findings of the assessment.
Please note that after the Initial Assessment has been carried out, we may decide that we cannot provide any treatment for example, treatment for the condition may not be suitable or appropriate.
Treatment sessions following Initial Assessment:
- We will normally provide you with any treatment sessions only after an Initial Assessment has been carried out. If there is a significant period between an Initial Assessment and us providing treatment sessions; we may ask you to undergo another Initial Assessment.
- If we have not agreed the number of sessions to be provided, we shall provide sessions on a session-by-session basis. We will agree the date and time of the first session and any subsequent sessions by telephone, in person or by email.
- Some sessions may take place at your home. In these cases it is your responsibility to make sure that we have a safe place to provide treatment, free from hazards and disruptions.
Late arrival or cancellation:
- If you are late arriving at a session, the session will begin on your arrival and continue until the time when it is scheduled to end. If you wish the session to overrun, and we agree, then you may be charged at our discretion for the extra time we spend in providing treatment.
- You are liable to pay in full (session and travel time) for missed appointments or appointments cancelled with less than 24 hours notice.
- Your appointment/s will be scheduled according to your needs and to the best of our ability. We shall endeavour to maintain appointment schedules subject to adverse traffic congestion and / or weather conditions which may result in unavoidable delays.
- The allocated therapist will have agreed check-in times with a nominated contact to ensure their safety. You will receive a highly professional service, please respond in kind. Threatening or violent behaviour will not be tolerated. All input will be withdrawn but you will still be charged at the full agreed rate. If required, we will pursue criminal prosecution.
Costs of sessions and payments:
- Fees are tailored to individual needs, please contact directly to discuss.
- Any additional costs will be agreed prior to being carried out.
- Invoices will be emailed out upon receipt of any booking for Physiotherapy services
- All payments need to be settled by the time of the appointment when possible or on receipt of the invoice or additional interest may be charged.
- A late payment charge of 10% will be added to your invoice for any part of days 15-60 which remain unpaid. If the invoice is still outstanding after day 60, it will be necessary to pass outstanding invoices to an agency for recovery.
- You are personally liable to pay all of our fees and any surcharges incurred, such as any cancellation fees. Where payment for services is made through a third party for example: a legal team, charity funding or health insurance provider it is your responsibility to pay your balance and recover the costs with the third party.
